Eligibility
If you are a regular full-time employee of CPC Integrated Health, you are eligible to participate in the CPC benefits program on the first of the month after your date of hire. As a new hire, you have the option to enroll yourself and/or your eligible dependents in CPC's benefits program. If you do not enroll within 30 days of your initial eligibility period, you will not have an opportunity to do so until the next open enrollment period.

If you don’t enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.​

Eligible dependents include:

Your legal spouse or domestic partner (domestic partners must submit a signed affidavit and one form of supporting documentation, such as a joint lease, mortgage, bank statement, or proof of insurance beneficiary)

Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage. Dependents may remain enrolled in your healthcare plan until the conclusion of the calendar year in which they reach the age of 26.

Adult children up to age 31 (New Jersey residents only), if they meet specific eligibility criteria under New Jersey law (e.g., unmarried, no dependents, not eligible for other coverage). Additional costs and annual renewal requirements apply.
